For 3 decades, LASIK has been one of the most recognized names in vision correction, a terrific way to improve nearsightedness and astigmatism (among other things). But there is another technology that has been around for many years that creates amazing vision for these same individuals – EVO ICL. LASIK has been performed on millions of patients worldwide and has become a household name. Because of that familiarity, people usually first hear about EVO ICL as an “alternative to LASIK” – something to consider only if they are not candidates for laser vision correction.
But that description does not tell the whole story.
EVO ICL is not simply a backup plan. It is a modern vision correction option with its own approach, benefits, and ideal candidates. For people with nearsightedness, with or without astigmatism, it reduces their dependence on glasses or contact lenses without removing corneal tissue.
So, is EVO ICL an alternative to LASIK or a modern vision correction option that stands on its own as a first-line treatment? The answer is both, but for many patients, it deserves to be considered on its own merits.
What Is EVO ICL?
EVO ICL (Implantable Collamer® Lens) is a thin, flexible lens that is placed inside the eye, behind the iris (the colored part of the eye), and in front of the natural lens.
Unlike a contact lens, EVO ICL does not sit on the surface of the eye. Once implanted, it is not visible, and patients cannot feel it the way they may feel a contact lens.
The lens works with the eye’s natural anatomy to focus light more accurately on the retina (the film of the eye), correcting nearsightedness and, if appropriate, astigmatism. The procedure does not replace the eye’s natural lens.
How Is EVO ICL Different From LASIK?
Both LASIK and EVO ICL are designed to reduce dependence on corrective eyewear, but they accomplish that goal differently.
During LASIK, a laser reshapes the cornea by removing a thin layer of tissue so that light focuses more precisely inside the eye. EVO ICL leaves the corneal shape and thickness intact and instead adds a prescription lens inside the eye.
That difference matters. EVO ICL is excellent for patients who:
- Have moderate to severe nearsightedness (-3 to -20 diopters) with astigmatism up to 4 diopters
- Want a lens that can be removed by an eye surgeon if medically necessary or desired in the future
- Have corneas that may be too thin or otherwise unsuitable for laser correction
- Experience dry-eye related to contact lens wear
- Prefer an option that does not permanently remove corneal tissue
Why Patients Are Considering EVO ICL as a First-Choice Option
Some patients arrive at an EVO ICL consultation after learning that LASIK may not be right for their eyes. Others begin with EVO ICL because its features align more closely with their preferences.
It does not remove corneal tissue
Because EVO ICL corrects vision by adding a lens inside the eye, the procedure does not rely on reshaping or removing corneal tissue. This may be especially appealing to patients with higher prescriptions or certain corneal characteristics.
It is an amazing option for higher levels of nearsightedness
EVO ICL is designed to correct or reduce moderate to high myopia. In the United States, FDA-approved indications include eligible patients ages 21 to 45 with myopia ranging from -3.0 D to -20.0 D. The toric model can also treat astigmatism up to 4 D.
The lens is removable
An ophthalmologist can remove the EVO lens if necessary – for instance, if you develop cataracts and desire or need cataract surgery in the future. Although no vision correction procedure can stop the eye from changing with age, the ability to remove the lens gives EVO ICL future flexibility that corneal tissue-removal procedures do not offer.
It includes UV protection
The Collamer® lens material includes a UV blocker. This is an added feature, but it does not replace sunglasses or other recommended UV protection.
It may appeal to patients concerned about dry eye
EVO ICL does not involve creating a corneal flap or using a laser to reshape the cornea. For some patients – particularly those already struggling with contact lens discomfort or dry-eye symptoms – that distinction can be meaningful. However, every patient’s eyes are different, and existing dry eye should still be evaluated and treated appropriately.
What Happens During the EVO ICL Procedure?
Before treatment, the SightTrust team performs a detailed evaluation to measure your prescription, corneas, pupils, anterior chamber depth, and overall eye health. These measurements help determine whether EVO ICL is appropriate and which lens size and power may be needed.
During the procedure, Dr. Shatz makes a small incision and gently inserts the folded lens. The lens then unfolds and is positioned behind the iris and in front of the natural lens. The EVO design includes a central opening that allows the eye’s natural fluid to flow through the lens.
Patients typically return home the same day. Vision may improve quickly, but healing and visual recovery vary. Follow-up appointments are essential so the surgeon can monitor eye pressure, lens position, healing, and vision.
Is EVO ICL Right for Everyone?
No vision correction procedure is right for every patient. EVO ICL candidacy depends on more than prescription alone.
A comprehensive consultation may include an assessment of:
- Age and prescription stability
- Degree of nearsightedness and astigmatism
- Anterior chamber depth and other eye measurements
- Corneal and retinal health
- Eye pressure
- History of eye disease or surgery
- Pregnancy, nursing, medications, and general health factors that may affect treatment timing or healing
